Current students
Current students and students accepted for admissions, only. If you’re not signed into the student portal or Canvas already, you will be asked to sign in again.
Students not yet accepted
Students that have not yet been accepted for admissions, only. You will need to create a “Dynamic Forms” account to access the form. Detailed instructions can be found below.
STEP 1: Create a Dynamic Forms Account
Use a personal email (Yahoo, Gmail, Outlook, etc.) when creating your account. If you are still in high school, please note that high school firewalls are very restrictive and will likely block the required activation email the system will send.
Take note of your username and password — you’ll need both to access your form.
STEP 2: Verify your Dynamic Forms account.
Dynamic Forms will send a verification email to the address you provided, with the subject line “Activate your account.” After clicking on the verification email, your Dynamic Forms account will be set up.
Helpful hint: Check your email on the same device you registered on. If you don’t see the verification email, look in your junk folder.
STEP 3: Log in to complete the application.
The verification email from Dynamic Forms includes a link to continue to the form. Follow this link and use your verified username and password to log in and access the MPTC form. That’s it!
