Moraine Park Technical College students are responsible for knowing college academic policies as outlined in the College Catalog as well as the Academic Calendar. The College does recognize that there are times when a student experiences an extenuating or unexpected emergency or hardship that interferes with the student’s ability to attend and/or successfully complete classes that may warrant an exception or appeal. The form must be filled out completely and supporting documentation is required. At the time of a qualifying event, students are encouraged to work with their instructors to see if arrangements can be made to complete class work.

Tuition Refund Appeal
Tuition refund appeal requests are considered on rare occasion for legitimate extenuating circumstances. Extenuating circumstances are those situations outside of a student’s control. Refund requests made after the 60-day grace period will not be accepted and students are responsible for payment.
Late Withdrawal Appeal
Students may have an extenuating circumstance that prevents them from withdrawing before the last date allowed to withdraw from the class(es) and from successfully meeting the course requirements. Late Withdrawal appeals do not refund any tuition fees. Refer to Tuition Refund Appeal Procedure for appealing for exception to the Wisconsin Technical College System refund policy (Wisconsin Technical College 10.08, Wisconsin Administrative Code).
STEP 1: Create a Dynamic Forms Account
Use a personal email (Yahoo, Gmail, Outlook, etc.) when creating your account. If you are still in high school, please note that high school firewalls are very restrictive and will likely block the required activation email the system will send.
Take note of your username and password — you’ll need both to access your form.
STEP 2: Verify your Dynamic Forms account.
Dynamic Forms will send a verification email to the address you provided, with the subject line “Activate your account.” After clicking on the verification email, your Dynamic Forms account will be set up.
Helpful hint: Check your email on the same device you registered on. If you don’t see the verification email, look in your junk folder.
STEP 3: Log in to complete the application.
The verification email from Dynamic Forms includes a link to continue to the form. Follow this link and use your verified username and password to log in and access the MPTC form. That’s it!
