Transcripts and Student Records

Submit Transcripts to MPTC

Official transcripts are required for admission.

  • These must be sent directly to MPTC from your high school or their third-party processor.
  • Transcripts sent by students are NOT official. If you mail or email your own transcript, it will not be accepted.

If you are still in high school:

  • Ask your high school to send your current transcript directly to MPTC.
  • Check your high school’s website for instructions on requesting official transcripts.

After graduating high school:

  • Ask your high school to send a FINAL OFFICIAL transcript that includes:
    • Graduation Date
    • Cumulative GPA
    • All Final Grades

If you did not earn a high school diploma, an official GED/HSED transcript is required for admission.

  • These must be sent directly to MPTC from the Wisconsin Department of Public Instruction, or your state Department of Education.
  • Transcripts sent by students are NOT official. If you mail or email your own transcript, it will not be accepted.

Official transcripts are required for transfer credit eligibility.

  • These must be sent directly to MPTC from your college or their third-party processor.
  • Transcripts sent by students are NOT official. If you mail or email your own transcript, it will not be accepted.
  • College transcripts are required even if the class is listed on your high school transcript.

If you attended multiple colleges:

  • Ask each college to send your official transcript directly to MPTC.
  • Check each college’s website for instructions on requesting official transcripts.

If you previously attended MPTC and are returning:

How to Send Your Official Transcripts

  • Please review the transcript requirements above before reaching out to your high school, college, or other institution to request transcripts.
  • Transcripts sent by students are NOT official. If you mail or email your own transcript, it will not be accepted.
  • Most institutions send official transcripts electronically through a third-party processor.
  • Institutions (not students) can email transcripts directly to: transcripts@morainepark.edu
  • If unable to send an electronic copy, official transcripts can be delivered to:

Get Your MPTC Transcript

If you previously attended MPTC and are now returning:

  • You do NOT need to send your MPTC transcript – we already have your information saved on your student record.

If you need to send your MPTC transcript to another college or agency:

  • Follow instructions below for How to Request Your MPTC Transcript.
  • Your MPTC transcript is the official record of your grades retained from all campuses.
  • Transcripts are confidential and cannot be released without your consent.
  • All financial obligations to MPTC must be satisfied before your transcript is released. Students are notified via email if debt exists on the account.

How to Request Your MPTC Transcript

MPTC uses Parchment as its authorized third-party processor for transcript requests. To request transcripts and track orders, you must create a Parchment account (if you don’t already have one). Standard processing fees apply for each transcript requested.

  • This option offers transcripts to be delivered to the recipient via email or postal mail.
  • Rush delivery is available with additional processing fees.
  • This option offers you, or the person you designate during your order, to pick up your transcript at an MPTC campus.
  • You will receive an email notification when your official transcript is ready to be picked up.
  • A Photo ID is required to pick up transcripts.
  • Pickup is only available during business hours at the campus chosen on your order.
    • West Bend Campus (Enrollment Services – Room T103)
    • Fond du Lac Campus (Enrollment Services – Room C004)
    • Monday through Friday 8:00 a.m.–4:30 p.m.
    • Closed on holidays, weekends, and Fridays during June and July.
  • Official transcripts are placed in a sealed envelope. The transcript is no longer considered official if you open the envelope before delivering it to the intended recipient.

Get Your Unofficial MPTC Transcript Anytime

If you have attended MPTC within the past 18 months, you can access your unofficial transcript any time. Log into the myMPTC Student Portal then navigate to myMPTC > My Student Center > Unofficial Transcript.

Get Your GED/HSED Transcript

Enrollment and Degree Verification

Enrollment Status per Semester (Fall or Spring)

  • Full-time = 12 or more undergraduate-level credits
  • Three-quarter = 9-11 undergraduate-level credits
  • Half-time = 6-8 undergraduate-level credits
  • Part-time = 1-5 undergraduate-level credits

Enrollment and Degree Verification Requests

Our Student Records office can provide verification to outside agencies (i.e., banks, employers, etc.).

If you are an Outside Agency:

If you are an MPTC Student:

  • Please complete an Authorization for Release Form, or
  • Use your MPTC student email to send the agency’s form to records@morainepark.edu, or
  • Send a written request (valid for one-time use only) to our Student Records office including the following:
    • Student Information (Full Name, Birthdate, Contact Information)
    • Who to release the information to.
    • Purpose for release of information.
    • What education records you want released to this agency or person.

Voter ID Enrollment Verification

  • In accordance with Wisconsin Statutes Chapter 55.02(6m)(f) in the state of Wisconsin, a Moraine Park student ID card, when presented with proof of enrollment, will suffice for voter identification.
  • To receive a student ID card, students must be currently enrolled in a Moraine Park course and provide verification of identity at the Student Services Office on any campus.

Replacement Diploma

Request a replacement diploma for $15 by emailing records@morainepark.edu. Include the following:

  • Current Name
  • Name when Attended MPTC
  • Program Completed
  • Approximate Date of Attendance

Processing of the replacement diploma may take up to two weeks. The replacement diploma style and signatures may not be the same as the original.

Student Records

Educational Records

  • Our Student Records office maintains a record of student activity (handwritten, print, tapes, film, computer, or other medium) at the Fond du Lac campus.
  • We encourage students to review their unofficial record through the myMPTC Student Portal.
  • Copies of, or inspection of, educational records are available via written request to the Student Records office.

Student Authorization for the Release of Educational Records

  • In compliance with the federal Family Education Rights and Privacy Act of 1974, Moraine Park is restricted from disclosing certain information from your student records.
  • You may grant MPTC permission to release information from your student records to a third party by completing and signing the Student Authorization for Release Form.

Student Record Change Form
